When: An injury occurred, even minor.

Near Miss Form

When: An incident almost caused injury, but no one was hurt.

  • Double unclipped, re-clipped in time

  • Carabiner found unlocked

  • Harness not double-backed

Staff Safety Report

When: Maintenance, staffing, or safety concern.

  • Not enough equipment

  • Trail crossing safety issue
