Synergo Participant Agreement Form

All participants in a Synergo challenge course experience need to complete a participant agreement form. The following form outlines some of the potential risks associated with your participation and answers many of the questions you might have concerning your experience. It is the group leader's responsibility to collect and check all of the completed and signed forms and present them to their lead facilitator at the start of their program.
