Participant Agreement Form
This form needs to be completed by all participants
on Synergo's challenge courses. A parent or legal guardian needs to sign
all forms for participants 17 years old or younger. Please bring these
forms with you to your Challenge Course and hand them to your facilitator
at the start of the day.
